Power supply "rattling" ONLY when in a game

Hi all. I have an issue where my nearly 3 year old power supply (Ultra LSP750 750-Watt Power Supply) will start making a rattling noise that sounds like a card in a bike tire after extended use. This only occurs when I'm in a game/my graphics card is in use. I'm dual-screening, and if I alt-tab out of a game (even if it's still shown on one screen) or close a game, the rattling immediately stops.

I redid all of the connections coming from the power supply. It also feels cool on the bottom of the case, but the PSU itself is incredibly hot to the touch.

This started about a week ago with no changes to my system, and all of my other components seem to be working properly.

System specs:
CPU: Intel i7 3770 (no OC)
GPU: EVGA Geforce GTX 660 2GB
RAM: 2x 8GB Corsair Vengeance
Mobo: Gigabyte Intel Z77 ATX
PSU: Ultra LSP 750 750 Watt
OS: Windows 8 Pro 64-bit
Display: 2x Asus VS247H-P 23.6" Wide (1 HDMI 1 DVI)

And here's a link to the actual power supply, if it helps.

Thank you very much to anyone who takes the time to help with this!

EDIT: Apparently Ultra is a horrible brand according to the PSU tier list here. It had so many good reviews on TigerDirect that I didn't even consider it to be an issue. May just have to replace it entirely, then?

Solution
The Ultra is a pretty lousy PSU - customer comments are essentially worthless for something like this given that none of them are putting their PSUs on a load tester. You may as well ask TigerDirect customers if you have high blood pressure for all the good it will do!

It's likely that as your power supply has degraded, your fan is starting to go and the additional heat is the fan barely keeping up with the load placed on the PSU. Even if you can still RMA (I don't know your exact warranty situation), it's not something I'd recommend.

Solution
Tier Five
Avoid IMMEDIATELY. These units are highly unsafe to use. No such protections added, very thin gauge wiring used, false advertising and too much to list. Reference to a higher tiered unit for a better, money saving and a much safer unit. For your safety's sake, please don't order or pick one up for use in your system. These units are a potential fire hazard and could even kill you, let alone your system.

That is the ranking of that power supply.

Use the TomsHardware Power Supply Listing to find a quality power supply for your system. We recommend only buying power supplies that are in Tier 1 or Tier 2. I would consider any Seasonic or XFX power supply to be included in that list, since not every power supply made is on this listing.

It's not so bad - there's a cash outlay now, but you'll have the piece of mind in that your PSU is protecting your system!
