[SOLVED] Power supply slowing dying?

So I recently upgraded from a 20" 1600x900 monitor to 3 24" 1920x1080 monitors and whenever I play certain games in 1920x1080 resolution my computer ends up with my monitors flickering then the computer crashes with no BSOD and restarts.

Does this screenshot below mean my power supply is going bad?

My computer specs are as follows:
Dell Inspiron 5675
8GBs of RAM
AMD Radeon RX 560 Graphics Card
AMD Ryzen 5 CPU
I'm not positive on the power supply, it is OEM.

Software tends not to read the voltages correctly , check the BIOS.

Yeah, that is what I'm noticing.
Any reason why the monitor ports would turn on/off just a few seconds before the computer crashes?
I'm guessing the GPU is trying to get more power but its maxing the PSU out which would cause the computer to shutdown.

Could be the PSU starting to latch off , then it crashes.
