Don't worry about the anomalous 12V and 5V readings. They're clearly wrong, as the system wouldn't work at all without the 12 and 5V lines from the PSU. I have a similar issue on my computer where it gives ridiculous and impossible figures for both the 5 and 12V readings. Hardware Monitor isn't the most reliable measure of voltages for a lot of systems.
As for the package power reading; the CPU is only taxed as much as is necessary. If BF4 only puts so much load onto the CPU, the CPU will only use a corresponding amount of power. If you run a CPU stress test, that maxes out all the cores, you'll see a package reading far higher (well, presuming it's reading accurately).
