Pseudo-Amateur looking to upgrade - Could really use help.

I've been Frankensteining my computer for awhile now, and I'm sort of at an impass in terms of what I need to do next. This is what I have currently.

Storage: 1.8 TB mechanical HDD's. (1 single TB and 1 750gb)
GPU: Radeon HD 7850 1gb Sapphire GPU
RAM: 8gb DDr3 RAM
Monitor: 27 in 1080x1920 Monitor
CPU: AMD FX-8350 Black Edition + Hyper Evo 212 Cooler
Power Supply: Sentey 750 watt
Motherboard: ASRock 970 Extrem3 R2.0

I basically do a lot of gaming/streaming and am leaning towards upgrading the GPU since I feel like mine is fairly weak. However, even then, I'm not really sure what is a solid upgrade choice given my budget. At absolute most I can afford to drop ~$350 US. That is AFTER shipping/handling/tax. I know it's a tall order, hoping someone more experienced can give me some input. Thanks!

Get a new gpu and thats all you will need.
