PSU voltages help

Hi, my pc is having problems with GPU demanding tasks, it reboots when a program pushes the gpu too hard, the gpu is an GTX 690, no overclock.
So I decided to check my Psu voltages and found this

the 5v rail running at 3.5 volts
and the 12 volt rail running at 16 volts
What can I do?

What does the BIOS say the voltages are?
