Radeon HD 645 core edition keeps crasking and recovering

I have recently purchase a radeon hd 6450 core edition 2gb graphics card for my standard HP desktop. I install the graphics card and power up and everything is fine while their are no video drivers are installed I install the drivers and amd catalyst control and reboot. The computer will get these yellow pixelater squares all over the screen. It's will freeze and then fix it's self saying the graphics card has stopped responding and has recovered. I have tried everything and it's really important I fix this issue my computer specs can be found at the link below. Thanks

You might have a bad card or you might have a bad driver. But I suspect it is some type of software conflicts. Here is what I would do...
Uninstall the AMD driver by running DDU from safe mode: http://www.majorgeeks.com/files/details/display_driver_uninstaller.html
Reboot to Win and let it load its default driver. Then run CCleaner and do the Clean and Registry portions both. If it finds a lot of registry errors, run it again. https://www.piriform.com/ccleaner/download
Then reboot again and install the latest driver for your card and OS as found here: http://support.amd.com/en-us/download
(or use the one that came on the disk with the card. It may be a little older)
See if the issue is gone. If not, it then may well be hardware.
