Radeon hd 6570 gaming mouse horizontal lines black screen

Hello,
when i am gaming and using 3D applications i get this error where everything freezes and i get black lines through the mouse cursor. The screen then goes black, and recovers shortly thereafter, but while playing online games this is a large issue.. 4GB RAM, onboard radeon card, upgraded to radeon 6570 via PCI. Hooked up using DVI-D to the monitor that came with the PC. been searching for help on this a long time.. any suggestions???

Yes this is a pavillion. Sorry for the excessive info. The PSU is a Dynex DX400WPS (obviously 400W), which is supposed to meet the minimum. I'm not running any other cards or addons other than this and a lot plugged in usb but thats only 5v.. Could that be the problem? I've had this issue as long as i have had the card. Also i don't know if this will help but if i am gaming and watching a youtube video and this occurs, when it comes back the youtube video is just green but the audio still plays. Hope this helps, and thanks for the reply.

Yes i have updated it, still getting the issue. Is there anything else that could cause this issue? I was kind of thinking that maybe it was being picky because i had an onboard gfx card that im not using but this is why i ask because i don't know. The spec's for the card says 400W and in brackets 500 for some added technology that i doubt either my board or power supply support and is disabled in graphics settings. If i were to get a larger power supply how large should i go?

Alright i understand. So in order to get enough power to run my card AND crossfire, i would need a crossfire ready PSU as well. With a good brand at 80% effiecency i would have to go about 650W. Now, the question is how many pin connector do i need for this pc? uninformed i bought a hunk of trash lol. So going for a 650W crossfire ready PSU (does my motherboard support crossfire i have no idea?) And curious on the number of pins or etc as i have been looking online but do not want to purchace the wrong one
 
No, it's a simple board. No Crossfire. A good PSU can of course last you going forward if you are upgrading.

A 6570 probably needs no power connections at all, other than the slot it's in.

A video card like this 7770

So, good PSUs come with 6+2 pin PCI-E power cables, so that they can be used for 6 or 8-pin connectors.

A 650W PSU might have 2 or 4 such cables.

And of course we don't know what your problem is yet. You could just have a defective one.

Why does your display memory say 3822? That's more a shared memory value.

Click start button and in the search box type dxdiag and hit enter. After the little green bar fills up click the save all information button and save it to desktop.

Open the resulting file and select JUST the first 20-25 lines of the "display devices" section and paste those into here.
