Random Loss of Signal to Monitor

Hi, I my monitor randomly loses signal when my pic keeps running. I've recently started having this problem, about a month ago my monitor would turn of for a sec and then turn back on. However now it turns off everyday and it doesn't turn back on. I have to manually reboot my pc and then it sometimes loses signal again as soon as I turn it back on. I tried booting it in safe mode and it still crashes. Also tired using a different monitor but still get the same problem. I don't know what's the problem and would appreciate any help.

Can you tell us the specifications of your pc with make and model#'s of the various parts if you can.

It's a old Dell Precision T3400
Motherboard CN-0TP412
Processor Intel Core 2 Quad Q6600
RAM 4x N1GT72U8PB0BY-25D(1GB)
Graphics Card Nvidia Quadro 4000

It also lost signal when I was in bios
