Red squares, red lines, BSODs, or black.

Hello, a friend of mine is having a bit of an issue with his rig. And as the title says, what happens is while playing games, or even just booting up, it happens at random times, either red and black squares will appear across the screen, red lines, or windows will BSOD, and/or a black screen. And then the monitor will report no signal.
His specs are a Lepa 650watt psu, amd fx 8350, Not overclocked, asus m5a99fx, kingston ram, and a Zotac gtx 760. nothing is overclocked. And we had replaced his motherboard with the asus, he used to have a piece of shit gigabyte, and trust me it was junk. but we thought that was the issue. I guess not. But that thing had issues of its own so it had to go.
It must be the GPU but wtf is wrong with it? I have never ever had an issue like this.
 

Temps are all good. But the funny thing is, is that he doesn't have to be gaming when this happens. It'll happen just sitting at the desktop when the system is at idle. Could be PSU but I just doubt it. I know Lepa doesn't make decent stuff either.
