The cable itself *should* not be limited by generation - ultimately it's extending the port you're attaching to.
If the port is gen 3, it's an extension of gen 3. I'm sure some of the cheaper 'no name' options have some perceived generation limitations, but I can't imagine how that would be the case.

Yes, one cable per GPU - again, you're just extending the port. One port on the board + one cable + one GPU.
