RMA sapphire 290x (amazon)

I had my sapphire 290x tri-x, 8gb for about 2 months now, and it keeps crashing. I run games like GTA V and BF4 and they crash (rsod) in 40 minutes. I purchased the card from amazon and I have no clue on how to rma it. I have some questions:
How to I get the card back to amazon?
Will I have to pay a extra fee?
How long will it take me to get it back (qatar)?
Do I need the original package it came in?

Update:
I am going to ship it back to amazon in 2 or 3 days. I was over warranty, and they said that I can get a full refund, although I have to pay for shipping ($40). I am looking at the 980ti. I want to get it for a couple of reasons:
I want to get a card that will last for 5 years because if I go with the 980 I will likely upgrade in 3 years (vram issues).
