Samsung 960 evo dead again?

I recently bought a samsung 960 evo 250GB m.2 drive and after a week of use i came back to my computer one day and it would not boot even into the bios with the drive in the computer, i came to the conclusion that the ssd was dead and i had just gotten a bad one. I returned it to the retailer and they replaced it. Now not even 3 days later the same thing happened again.

Is this just bad luck or is this product just crap? should i get this replaced again with the same samsung ssd or something else.

This may not be relevant to the thread but I have tried everything i know but nothing seems to help me.

I got Samsung Evo 960 few days back and cloned my boot drive to it using Samsung Magician, to use on my Acer VN7 592g 767q. It worked for a day and after a BIOS update from Acer it wasn't recognized as the boot drive and in Windows as well when i booted laptop after the BIOS update. The SSD is recognized as a boot device it the BIOS but when I try to clean install Windows on it , its not recognized in the installation page or Disk Management in window and diskpart doesn't list it as a drive as well.
If anyone can help me to get it formatted while its in M.2 slot or can I rig up a SSD adapter and a PCIe riser from below to use...
