Screen freeze, spike, splits while gaming.

Recently, my pc ran into trouble. When I start to play a game on it, screen freezes, splits and black horizontal lines appear.
Sometimes, OS is working and I can quit game and switch to dekstop. Screen remains splitted with black lines. After 15-20 minutes , I get a notification that GPU driver has recovered, and screen gets normal again. When I start to play game, it happens again. Happens with all the games.
Other times, full system gets hanged and I have to take out plug to restart it. When it restarts everything is fine.
Already did a graphics driver update. Uninstalled and reinstalled the latest driver.

Sapphire HD 6870
Intel Q8300
4GB DDR2 800MHz
Adata 64GB SSD
WD 500GB HDD
iBall 500W PSU.
 
Solution
First thing I'd do is run Guru3d's driver sweeper, then re-install the drivers. I know you said you uninstalled and re-installed but this is more thorough and effective. Assuming it still persists, which i'm betting it does, I think it's time for a new GPU.

Solution
Not the best time to buy into Sony haha. I actually had a ps4 and a Xbox one, ended up selling the ps4. Loved the controllers and the build quality, but to me the graphics difference was in noticeable, and if I wanted something to look good, I get it on PC. So given the xbox's extra features I chose it. Not saying its better then the ps4, just giving you a probably less considered viewpoint. The ps4 was nice, but I just ended up not buying more games for it.
