Screen goes black while a session of gaming , no signal, screen shuts off after the restart also

hello guys,
i have this problem on my PC where after i start gaming for example CSGO it turns black and the screen goes to power saving mode, when this happens the sound stutters for a few seconds ( the sounds gets kinda stuck and repeats itself). after this i restart the PC and it turns on and everything is fine , however this problem can repeat itself a few times , for example it happens i restart my PC and relog to CSGO and it happens again and rinse and repeat.
the times which it occurs at are random ( it can happen at the second round of a CSGO match which is about 10 minutes in to the game or it can happen 40 minutes into the game or it can not happen at all )
also there are times when the problem happens and i restart my PC (to fix it) and the same thing happens right after i log into my PC and try to start an app and this can repeat a few times.
there was this time when the problem happened and i restarted my PC and went away to make coffee and stuff for about 5-10 minutes , when i got back i logged into my PC and the problem happened (even though i had left the PC for a while after the problem happened happened on the first time it repeated).
it can also happen when i alt + tab in and out of the game( for example once i tried to close a norton "high cpu usage" popup and it opted me out of the game and when i opted back the problem happend) .
sometimes when i am on my desktop(after restarting due to the problem) the problem is
accompanied with a USB disconnection sound.
i had tried reconnecting the graphics card on the PCIE slot and reconnecting the power cable and run memory tests and i also tuned up my gpu fans to the max.
also this only happens while i am in game.
my pc:
i5 4960k
16 gb ddr3 ram hyperx
gtx 960 4gb oc edition by msi
550 wat power supply
i bought it pre built and it worked fine for about a year.
it started happening once or twice a week and now it happens very often.
how do i fix this ???

Exactly but the same programs used to test an overclock can be used for a default system that never was overclocked just to see if its stable at defaults

In fact forget about prime IMHO its obsolete, Linpack is better, OCCT has it integrated, run OCCT linpack 64 bits with AVX and 90% memory usage, let it run min 24 hours, if it detects an error it will squeal and stop with a red alert. IF it does show an error your CPU most likely is bad, if it doesnt run the GPU test and finally if both pass run the PSU test which combines the 2 earlier tests for max power consumption.


well i never over clocked my cpu (may be the store i bought it from did ) but i never did anything to it.
could you elaborate on what i need to do, i don't really understand the terms you've used there.

Ok , when i have the time i will run linpack 64 for 24 hrs
