Sd card detected by pc but cant format

My sd card is not getting detected by my android phone,when inserted it says sd card blank and that it is damaged.
When inserted using a card reader in the pc which runs windows 7,it gets detected but i am not able to open the drive and also im not able to format the drive,when i try to format it with computer it keeps saying "windows was unable to complete the format"..
pls help me with this and also i have a Sandisk 8GB micro sd card

its already a raw file system go to disk management and you can freely format it format it to FAT32 file system
