Seems Can't Delete File From My SD Card

I Have A 16 GB MicroSD Card. When I Delete Something From The card on the computer, then i put it back to my phone the files that i deleted is came back up in my card. I tried many things to delete the files from the SD card, i tried to format the disk but the windows is unable to complete the format. I tried delete the files from m phone too, but when i restart it or i unmount the sd card the files is always came back up!
Can Someone Help Me?!

Before removing it from your PC, make sure you dismount the device. When in the computer, are you using an adapter that has a write-protect switch on it? Make sure it is not in the "Safe" or "Read-Only" position.

yes i'm using an adapter. but the switch position is not in the "Safe" or "Read-Only" position. i already do everything, i think.
