Question Signal lost while gaming or when just starting a game

hi guys its really annoying when ever i am playing i am getting this issue whether in mid game or just when i start one.

here are my PC specs:
i7 4790k
HyperX Predator 16GB
MSI GTX 760 2GB
550w PSU

also had a weird problem its the top fan making loud noise for long time when starting the PC dont know if this two issues have something in common thanks in advance for the help.

got a temporary fix by setting vsync in nvadia control panel to adaptive in global setting tab im runing 1080p 60fps
