Someone help please

I recently built a PC. It's about 2-3 months old now. Recently I haven't been able to play games. I figured out that the disk was running at 100 precent. I've been looking online and nothing. I've done everything I've seen and it's still doing it. Can someone please help cause I just wanna game man. Edit: could it be that my disk is broken? At this point idk I just wanna know if I can fix it and how.

astrogamerchaos, download and install SeaTools and run the extended test (it takes a long time) and that will let you know the health of the drive and if it is cause of your problems.
