There are two onboard USB 2.0 headers for the Soyo KT400 motherboard. I
usually use one for the two usb ports on the front of the case and the other
one is empty.

On Friday I installed a 4 USB port PCI card hoping to have a total of 6
ports on the rear and the two on the front ofthe case. However the onboard
headers no longer seem to be working.

Any suggestions ?

the PCI USB controller is seen by the OS and the on board controller
has been disabled, either in the BIOS by you, or the OS has disabled it ~

It is not disabled in BIOS so what/where should I look for in the OS ?

sorry for the delay, look in device manager; I may be wrong, but I
dont believe you can have two USB controllers with one computer ~
