SSD problem after swap

I swapped my pc case, booted up fine. I didn't notice I had a problem until I noticed some of my steam games weren't installed. I started from the bottom, connections are fine. I went into the bios, it's detected. I went into device manager, it's in there and apparently in working order. I went into disk management and it's also in there, but I can't open or explore it, my only option is to delete. Which wouldn't be a huge deal if I knew what was being deleted.

I set it up so that both disks combined to one C: partition, it use to show up as 256, so disk0 is definitely not appearing
 
how? It doesn't give me the option, its faded out. Also, I want to point out that disk1 has a 100mb efi partion while disk0 doesn't, but it does have 128 mb of unlocated free space. Is that not odd?
 
Let's not play the guessing game, I've included all the info in my original post. How is that efi partition created? I tried to make one just now but I can only format standard ones like fat and ntfs. Perhaps from the bios??
