Question strange issue.. HDD activity stuck at 100% with high response time randomly, unless I'm running a scan

I'm facing a weird issue... my hard drive active time gets stuck at 100% for a while and I/O speed drops from MB to KB making my pc slow.. this problem goes away if i ran any type of scan.. at this time I/O speed remains at MB, high response time process goes away and pc runs fine.. what could be the problem?

things i've tried:

1- scanned my pc with anti-virus and anti-maleware
2- run a CHKSDK for bad sectors
3- re-installed my GPU driver with a new one
4-used CCleaner to remove pc trashes

My PC specs: Windows 7 Ultimate - 8G Ram 4 x 2 - intel i5-3470 - Nividia GTX 1060 3GB - WD 1TB Blue - Gigabyte H61m-S2p - HEXA+ 500W PSU.

If it's a hard drive, that'll happen. If you're playing games or doing anything resource-loading intensive, you may notice stuttering.

games runs fine.. i get this problem while im browsing, opeining vlc, etc..
