Strange issue with mouse pointer speed

EDIT: The game I was playing (Dragon's Dogma PC) turns off mouse smoothing while running for whatever reason and didn't change it back when it crashed.

You "checked" the "mouse driver". Did you try reinstalling the drivers?

The crash may have corrupted something.

Reinstall the mouse driver software and go from there.
