Stripped Screen Crash

So i got a new gaming computer the last summer and everything was fine until 2 or 3 months passed then my screen got a lot of stripes( every crash had a different color). I wasn't getting it to frequently but as time passed it got worse. i checked my graphics card and it was very hot and i put something very cold on it(not water or ice) and i could play without problems until now. when a try to play games, especially when playing dark souls 3, its crashes from 20 to 20 minutes.Could it be that a don't have some drivers installed correctly?(my keyboard came with a CD that i think there where the drivers.. and i also play with a PS4 controller using DS4).I am not very familiar with computers, so i didn't really knew all my specs but i used a site(My Computer Details) and here is what it said :

Processor Intel(R) Core(TM) i5-4690K CPU @ 3.50GHz

Video Card Intel(R) HD Graphics 4600

Video Card #2 AMD Radeon R9 200 Series

RAM 8.0 GB

Operating System Microsoft Windows 7 Professional Edition Service Pack 1 (build 7601), 64-bit

These where the basic tab but i can give you the advanced specs but it was way too long.

I also have a pretty old monitor, i think its 3-4 years old.

Those stripes are artifacts which 99.9% of the time indicate your graphics card is dying.
On a side note, never use a nonconventional method to keep your GPU cool, it's just not ideal.. Invest into/ setup better airflow management into your case ( more case fans, etc )

Also, please list your temps when these artifacts actually occur, and give us your actual graphics card name , not just the r9 200 series

Can you tell me where could i find it( the graphics card name) and how do i actually measure the temperature when the crash occur? i didn't build my pc and i don't really know my specs to well


And also, is there i way i can make it so it doesn't crash so frequently? I will speak like the next month or so with the guy that made the pc about a new graphics card, but i still wan't to play.

and would it be helpful if i would make a photo with my pc?
