[SOLVED] Sudden screen crash. Require new GPU?

Hi folks.
Over the past two days I've had this weird screen crash

It seems random, although it happened twice after I had finished gaming, the screen suddenly artifacts and then immediately goes into a gray screen with vertical lines. What's strange is, if I hard reset the pc, the screen starts functioning again.

EDIT: I switched to integrated graphics, and so far I haven't had any more crashes

Possible reasons:

1. Old hardware, my GPU is about 6 years old and one of the fans has stopped functioning properly. Although monitoring my GPU temp, I don't believe it went above 60-70.

2. New PSU and Case. About 2 weeks ago my PSU died and I bough a new one along with a new Case. Meaning I dismanteled everything and rebuilt it . PSU possibly faulty or a mistake in building process?

Here are my specs.

Windows 10 Pro
CPU: intel i5 4670k
GPU: Radeon r 280x
PSU: Corsair RM650 ( Previous was Corsair TX650 )
 
Solution
Power supply is good model, need to test the video card in another system with a good PSU and/or another video card in yours before you can check for any other things.
