[SOLVED] The witcher 3 Keeps crashing?

Hello everybody,

I have recently built my own PC which is a first for me and everything was going very well until i started playing the witcher! The gear in the PC is

ASUS AM4 TUF Gaming X570-Plus (Wi-Fi)
Ryzen 5 3600X
AMD RX 5700-XT THICC III Ultra 8GB
Corsair Vengeance LPX DDR4 3000 C15 4x8GB
Intel 660p NVMe PCIe M.2 1TB
Cooler Master MWE Gold Fully Modular 750W
MasterLiquid ML240L RGB AIO Cooler plus a few other fans to pump air out of the case
Windows 10 64

so i started playing the Witcher a few days ago on the max settings which worked Perfectly for about 15 hours and now all of a sudden it just randomly started freezing when i play! Most of the time its when i'm in the inventory menu it will just freeze on the screen so i then have to go back to the desktop and close it, A couple of times the screen has gone black but all the sounds and music continue to play through the speakers, And one time at the beginning of a cut scene it closed the whole game down and went back to the desktop? i have tried turning the graphics down to the low settings which didn't work, i followed some google advice and installed a 6 month old driver for the graphics card which didn't work, i turned off vsync, i set it to unlimited frames, i set it to full screen.

I have just tried "This patches x86 executables in order to let them have 4GB (instead of only 2) of virtual memory on x64 platforms" which did seem to work for a few hours without fault and now its worse.

It's also crashing at the beginning of all the cutscenes now and closing steam down completely with these error codes

"create render target view failed can't create d3d window"

"EXCEPTION_ACCESS_VIOLATION (3221225477), Error reading location 0xb51915ac"

i ran some online tests and got these results

UserBenchmarks: Game 107%, Desk 125%, Work 100%
CPU: AMD Ryzen 5 3600X - 91.9%
GPU: AMD RX 5700-XT - 108.9%
SSD: Intel 660p NVMe PCIe M.2 1TB - 267%
RAM: Corsair Vengeance LPX DDR4 3000 C15 4x8GB - 66.4%
MBD: Asus TUF GAMING X570-PLUS (WI-FI)

I'm not sure what else to do now, Everything i have tried seems to be making the whole thing worse!

Any help would be greatly appreciated and i thank you all in advance for any information you have.


Update - So I just changed drivers again as the last one seemed to be making it crash instantly and have taken a photo of the performance overlay during a cutscene that it was crashing on if this helps anything?

Start with the simple solutions, but first back up your save data just in case:

I'd recommend doing a verify integrity on the game itself through steam, games can get corrupted.

Hope my simple solution works.
I did try that one and it did find 2 issues with some files which it replaced but it didn't seem to do the job sadly. I had backed up the steam file just in case which I assumed was the save games but apparently not which is lucky so thank you for that link.
