Do tokens go to the graveyard (and then cease to exist)???

Could I e.g. equip a Pentavite (i.e. a 1/1 artifact creature token
from a Pentavus) with a Skullclamp and draw two cards (because of
Skullclamp's triggered ability)?

I would say, the token goes to the graveyard (CompRule 217.4a) and
then ceases to exist; but that's enough to trigger the Skullclamp's
ability (CompRule 216.3).

Yes (and yes), assuming something sends them there. (They generally don't just
fall in there on their own at some point.)
