It really seems like the flash drive has failed. It's not uncommon for flash drives and SD cards to lock themselves like this when they fail. If you've tried the Regedit, Diskpart and other tools, you've already exhausted all of the options. :\

I'd recommend that you RMA it if it's still under warranty.
