Right. Trinisphere asks "Does this cost less than 3 to play?" once you figure
out what the cost should be; for an X=4 Fireball, sitting on the stack because
that's where it went in 409.1a, you figure out in 409.1f that it should cost
4R to play. Trinisphere wakes up, says "That's not less than three", and
goes back to sleep, doing nothing.

Correct. In this case, Trinisphere says "Wait, that only costs 1R, add 1 so
the total cost adds to three".
A Fireball _off_ the stack has mana cost R and converted mana cost 1... but
while it's a spell, it's NOT off the stack, and anything looking at it sees
the full value of X.
