[SOLVED] Two Identical LSI Controllers Not Working Together

So I have two IBM M5015 controllers (rebranded LSI 9260-8i)
Both cards work perfectly installed in the system individually, I can create RAIDs / LUNs and mount and use them without any issues.
The problem starts when I install both cards together into the workstation.
I get his with this error code

It doesn't matter which card loads first, the second card will always be the one to show this error

F/W is in Fault State - MFI Register State 0xFFFFFFFF
F/W has indicated a system reset it required. System will reboot now ...

I'm running out of idea's here, so here's a list of what I've ruled out so far
  1. Upgraded the cards to the latest firmware, both are running the same
  2. Tried different PCIe slots
  3. Removed any other PCIe devices (other than GPU)
  4. Swapping the positions of the two cards
  5. Tried disconnecting all the drives from the cards
  6. Disable PCIe power savings in BIOS
  7. BIOS is on the latest version

Fixed it
The cards config utility only worked in legacy mode, but disabling legacy mode and setting the cards to "Ignore Error" mode seems to have got them to boot finally!
