Unstable gaming performance

So I am having some annoying FPS/usage issues while playing games.
There are two major problems
1. Random GPU usage drops ( from 99 % to 20 %) which leads to heavy FPS drops.
2. Low GPU usage all the time ( 50 % or below )
This has been happening for over 2 years. At first when it happened I had R7 250 and an AMD Athlon II X2 245 processor. So I thought maybe the problem is with the processor. A few days later I upgraded my processor and RAM to i3 4150 and to 4 GB DDR3. I played Pillars of Eterntiy and was getting only 20 FPS. Here's something interesting though FPS reduced when more NPCs appeared or any of my teammembers. So if the game was running at 60 FPS it reduced to 20 FPS in cities and populated areas ( though this should not happen since the game has very low requirements ). This kind of problem varied from games to games. Either it was low FPS or random FPS drops or both. I had no problem when I was playing MGS V The Phantom Pain, Dragon Age Inquisition and Battlefield Hardline. But games like Far Cry 4, Arkham City had random FPS drops. I checked the CPU and GPU temps and both were below threat level. Some games showed this problem some games did not and it didn't matter if the game of 2D or 3D. But let me come to the recent days. Now I have 8 GB RAM and GTX 750 Ti.
So now it even get more strange. I started playing Dark Souls 3 but the GPU usage was around 30 % and FPS around 20.I started recording using FRAPS and then, to my surprise, the GPU usage increased to 99 % and FPS to 50+. Then I started recording games while playing and it did help in some but in some it did not work. I played games like Firewatch, Arkham Knight, Everybody's Gone To Rapture and they all had low GPU usage and random FPS drops. A few days later I reinstalled Windows 8.1 over the older one and for some time it fixed the problems but they resumed after some time. Then instead of reinstalling windows I formatted my C drive and reinstalled a new one and doing this thing fixed my problem for like 3 months. In these 3 months I was getting neither low FPS nor random drops. I played Tyranny, Doom, Deus Ex Mankind Divided, Divinity Original Sin 2 and a few more wothout any problems. So for some time reformatting harddisk and reinstalling windows did help. But now nothing works.
These are the games I recently played
Gears of War 4 - No Issues
Far Cry 5 - No issues
Just Cause 3 - Random FPS drops
Pillars of Eternity 2 - Low GPU usage
Dark Souls Remastered - Low FPS but fixed it when I started recording
I also thought maybe I should try running Tyranny and now I am getting only 26 FPS on the game instead of 60.
This problem is annoying but not able to understand the problem is even more annoying. Did anything happened to any of your system ? Does anyone have any solution ?
PS: In my place power loss is a common thing and I don't have any backup so system just shuts off. It has happened a lot and when the problem isn't there before power loss it sure is when I turn my PC back on.
Also. many a times in task manager my hard disk usage shows 100 % even when I am not installing or copying anything.
