USB 2.0 PCI card with front panel connectors?

Hello,

I'm looking for a USB 2.0 PCI card with an internal header pinout so I can plug in the front headers on the case to the card. The reason why I can't use the motherboard's header is because the onboard USB 2.0 controller is fried for some reason and will not work.

This is an LGA775 board and it does have PCI-e ports. And I am willing to modify the USB 2.0 PCI card that's already in it right now.
