USB 3.0 Header Control

Is there a PCI-E card with 3.0 connector that you can install that will support and control all ports of a 7 port USB 3.0 front panel hub? Would like to add a front panel hub but need all USB ports to work since getting to the back of the computer will be a pain once positioned.

How many USB cables are required by that front panel hub? 2, 3 or 4?
