Usb Devices shutting down in windows startup

Hi,
When i turn my pc on all the usb devices light up and i can access the bios and my keyboard and mouse work.
When i load windows all the usb devices stop apart from 1 generic mouse works if it is in the correct front usb 3 port.
I have attempted to reinstall windows but when i load the windows disk all my usb devices fail!
I have had a replacement motherboard sent to me and I still have the same problem.

Here are my specs,

Intel 15 4670k
ASROCK Z97 Extreme4
16gb Ram
Gtx 670 4gb
250gb Samsung SSD (Operating system)
480gb Ocz SSD
2TB Seagate SSHD
2TB Seagate HDD
Corsair CX 750 power supply
Windows 7 Ultimate 64

Fortunately i can access windows with the on screen keyboard but i have no success reinstalling the drivers for the motherboard or any other device.
I have tried the usb settings in the bios without success.
I havent updated the new motherboard yet but i updated the first one and it made no difference.
I tried removing the SSD with Win 7 completely and i still cant reinstall windows because of usb failure.
The pc seems to work fine apart from the usb issue. I have been using steam in home streaming from it and accessing files from my other pc fine.

I am at a loss at how to proceed now apart from trying a 3rd motherboard which seems an unlikely fix at this point.

Any ideas would be much appreciated.

I am not sure what to look for with my voltages.
H/W monitor reports:

Cpu input voltage +1.744 V
Cpu Vcore. +1.208 V
Cpu cache voltage +1.072 V
System agent voltage +0.864 V
Cpu analog IO voltage +1.008 V
Cpu digital IO voltage. +1.008 V
+12.00V. +12.096 V
+5.00V. +5.064 V
+3.30V. +3.328 V

When I contacted ASROCK about the fault they suggested I try removing the gpu and all USB apart from the keyboard and mouse. I assume this was to test if there was adequate power.

How old are your USB peripherals? I have experienced USB device failures in the past that effected the entire system. In one case it was a mouse, took ~3 months to figure that one since it wasn't consistent in how and when it would act up. If possible, I'll suggest swapping out the mouse and keyboard to see if conditions continue
 


I have a Roccat Kone and Roccat Isku plus a cheap generic keyboard and mouse which all work in the bios, then by the windows login screen the only thing that works is the cheap mouse.
I have checked the keyboards and mice and they work fine on my other system
Any other ideas?

I purchased the motherboard, processor and case only a month ago. I sent back the first motherboard and I have only had this one a week.
It seems to have exactly the same USB problems as the first motherboard.
Maybe I am just very unlucky and should try a third replacement?

I'm thinking that if the problem doesn't follow the peripherals, it isn't them. There are quite a few threads about Roccat and ASRock not playing nice together but this seemed to be something worth pursuing... Thanks I seem to have made some progress with this now.
I disabled smart connect but nothing seemed to change. Then i enabled fast connect in the bios and got an error boot mgr missing.
I booted from the windows disk and the keyboard and mouse stayed active so I can now reinstall windows finally!
I'll let you know how it turns out...
