USB Ports and Video Card don't always power

A lot of the times when I boot my computer, the USB ports don't power properly. They will get powered when I first boot the computer, but when it goes to load Windows 7, they will get unpowered and maybe 1/4 of the time they get powered. Every once in while my monitor won't receive a signal, which I assume means the video card isn't getting powered correctly. I've updated my BIOS to the latest version. Any suggestions? Could it be the PSU not having enough watts? Could it be my mobo? Could it be related to the Haswell USB 3.0 bug even though some of the ports are USB 2.0?

Did you install all the latest of the motherboard drivers from the site of asrock?

From what i read it otherwise probably is the motherboard and you would have to go for warranty.
