USB Won't Format

So I have a 64G usb and it started playing up the other day. There were errors in everything I copied in it. Additionally, when I tried to delete from it, it would say that that is write-protected, so I formatted it. It turned out with an error, saying, "Windows was unable to complete the format." Now it shows up in This PC as a removable disk and tells me to format it when I go in. I have tried:

1. Format from Disk Management

2. Use diskpart to clean, create partition and format. (also failed)

3. Tried using Mac's Disk Utility. It wouldn't finish the format. The progress bar hanged at half and time kept increasing.

4. Use EaseUS Partition Manager Free to format. It me an error message, which said,"There are some error occurred while writing partition chains on disk."

Are there any other ways to fix this?
