Using one headset for PS4 and PC

Hello,
I have Logitech G430 headset. It has two 3.5 mm jacks, one for headset and one for mic, however it also comes with a USB adapter.
What I want to do is to connect this headset to PC and PS4 at the same time. I would like to use both the mic and the headphones on both devices if possible, but mic only on one of them is also an option.
So what are my options? I know some video game streamers do this but I don't know how.
Do I use an external mixer or anything like that?

Great budget built. The i3 paired with the gtx 960 will rock any game you want to play even at a low price. ( Just a note the ram wasnt on the pcpartpicker database so I had to link it manually. Its a special deal which is only $38 for 2x4gigs of ram that's pretty amazing)
