Video Card Overtemperature

hello everyone!
I have been monitoring the temperature of my 2 years aged card(VisionTeK Radeon HD 6750) and it reaches 95*C with most games, I tried with a similar of my brother (Asus Radon HD 7750) and it does not happen.

this is my setup:
Asus P8Z77-V LX
Intel Core i3-3240
2x4Gb 1600 G.Skill Ripjaws X
Cooler Master GX 650W 52A
2 HDD Sata6Gb/s
1 SSD
Windows 8.1 x64

can someone tell me what's happening??

Clean it, if clean, remove the cooler and re-apply thermal paste. Ensure your chassis has enough airflow.

I forgot to say the videocard has a 95% of usage while working. Does it change something?

No, it does not.

Yes, I applied a Cooler Master Ice Fusion thermal paste, and works cooler reducing 40*C.
