there is a full village where-in the village elders believe the future is not good for children ( due to latest technologies and corrupt practices) and they cut off themselves with the outside world. The kids grow up in that setting for many years.
A monster (a village elder in disguise) comes intermittently to trouble the villagers. This monster (it is shown) normally stays outside the village that connects to the next village. Due to a medical emergency / inter faction fighting, one kid (son of the elder) is forced to go to the outside world to get the medicine for treating his family. it is then revealed that the outside world is a busy US city ( with fast moving cars and inter state roadways ) . and he is stunned to see that.
