Visual lag despite high fps

Hi, i've been having this really annoying issue in games where my gameplay is completely unsmooth despite the fact that i'm playing 150+ fps most of the time.
turning vsync on or caping fps to 60 does not help, it actually makes it even worse.
specs:
CPU: i5-4460
GPU: Nvidia GTX 1060-6GB
RAM: Kingston HyperX DDR3 1600 C10 1x8GB
MBD: Gigabyte GA-H81M-D2V
i tried doing a fresh install of windows a couple of times, only one or two out of those times the problem stopped for the first day of playing but it keeps coming back. the temperatures of cpu and gpu are fine, drivers should be up to date, i'm not sure if it;'s a problem with the monitor or something else

HP Pavilion 23bw, overclocked to 76hz and an hdmi cable

 
So that's an older 1080p monitor, thus doesn't matter if the cable is HDMI 2.0 or not. You may have to set it down to 60hz in case of a possible instability issue. Also that monitor has a 7ms response time. Ideally you want a monitor with a response time of 1ms - 5ms(1 being best)... so that could also be part of it.
 

well i'm buying a new monitor anyways, for now i'll try doing what you said and set the hz back to 60, thanks for the advice!
