ways to remotely disable a mobile phone

My iphone 5s was stolen and for some reason the theft managed to crack my apple id so she was using it freely. My question is, if i know the phones ip/mac addres, is it possible to disable it and prevent her from using the unit any further? Please help.
 
This is why I like to recommend people install Find My Phone (free and from the app store) on all of your Apple devices. Not only can it track your device location, you can remote wipe and/or shut down as well as a host of other features.

Can you disable it directly with the phone's mac address? No. Not legally, anyway. Your best bet is to talk to your local police department and to Apple customer support. Unless it was stolen by your sister, then please, speak with your parents.

If the thief was able to crack your Apple ID, either the thief is good, or your passwording skills are bad. Either way, you've lost far more than your phone. It's borderline identity theft. I hope you didn't also use Apple Pay.

Regardless, unless the thief is going to sit around brute forcing that passcode open to actually use your phone... wait, did you even have one?
