Have you overclocked anything on your PC? CPU, GPU, or RAM?

This is a fairly common issue especially when overclocking the GPU. If you have pushed it too far without enough voltage, or are just pushing the limits of your card, this could be the cause. I would recommend lowering your clock speeds on the GPU if this is the case. You may need to lower them even if they are at stock to take care of this issue. I'm not as familiar with the RX 480 as I am with the 580, but they do have some power delivery issues.

As always, make sure you have the current drivers for your RX 480, and make a clean install of them if you do not. Here's a link for a driver removal tool:
