Weird HDD issue (Seagate)

In 2008 I bought my computer which came with my Harddrive (Seagate ST3750630AS 750GB).. Now im not really a tech savvy guy, maybe slightly more than the average 20 year old but my HDD has been acting weird ever since I got it.
Once a month or so it would start working extremely slowly, anything that had to be loaded from the HDD was taking ages to load, watching movies while this happened made the video being forced to buffer every few seconds and it would take 20 minutes for a game to startup. This would last a few hours and then it would be alright again, at the time I thought it was just some system check or something. I didnt know that it was the HDD causing this to happen. So this issue has been there ever since I got the computer.
When SSD got released I bought a 120GB one and put boot, system, everything on the SSD, wiped the HDD clean and used it purely for storage and my problems were solved.
Recently however, everytime my HDD got crazy it started to affect things that are in my SSD, Google Chrome, World of Warcraft, anything. If things are loaded in the RAM then things run perfectly fine but as soon as I try something new everything slows down. Technically, if I unplug the HDD right now, nothing should be affected, but looking at the resource manager, everything would shut down, even Windows is affected by this.

I used HD Sentinel to see what was wrong and this popped up:

(The PORN!!!!!!!! is my backup partition with important stuff incase i have to format SSD, dont get naughty ideas :p)

I have no clue what to do and I rather not format my computer yet.

I'd suggest you to try testing your HDD with the manufacturer's specific diagnostic tool, you should be able to find it on their website. It will be best to benchmark both SMART results. However, I'd strongly recommend to back up all your data from the drive before proceeding with the troubleshooting.
I'd also try plugging the drive in a different SATA port and using a different SATA cable, then perform the tests again and see if there is any improvement on the SMART attributes. It seems like you have 6 bad sectors which is not that bad; the spin retry count, however, is a bit concerning since it might indicate that there are physical parts on your HDD, that have started degrading or malfunctioning.

Back up any important data just in case and check in with your HDD manufacturer's tech support to see if it's still covered by the warranty. If it is, you could RMA it and get a replacement.

I will do the diagnostic tool tomorrow, im assuming you want me to do the diagnostic tool that is bootable rather than the windows one.

But this issue has been here for 7 years now so what im also wondering is why my HDD, which is purely a storage drive with no system, boot or anything installed, is affecting Games in SSD. Example if im in a loading screen my HDD performance goes up to 100% while SSD isnt.

It might be something in the connections to the drives. I'd suggest you to run some testing utility on the SSD and check up on the performance with the secondary drive plugged in and without it.
