Weird patterns on screen

Color fades and weird geometry/symmetric yellow patter on screen. I know it is gpu related because it occurs only when i open games..
Screen freezes, if I press power button then it shut downs normally and shutting screen is shown. Any way to fix it? Already tried re installing drivers ftom nvidia site for gt 630

Replace the graphics card. It appears to be damaged.

It was running fine from months. Suddenly this happened. Also I know it's gpu related . should i just throw it and buy new?

If your system doesn't have adequate cooling and you're gaming a lot, then graphics card can overheat and go bad.
Yes - replace the card. And increase cooling of your system.

Temps seem fine.. Idle temps of cpu 20C under load 40C max. Gpu doesn't go above 50C. In take and exhaust fans working. Pc is clean too.
