Hey this is my first post. I've been working on this problem for about a day now with no luck.
My problem started when I was having 100% disk usage in Windows on SSD that was causing my computer to have major lag. I ran chkdsk /r /f from the recovery partition multiple times and each time it came up completely clean. I installed some software (Visual Studios 2015) overnight and just let it sit overnight. This morning I go to check on it and there is a flag that suggests disk maintenance. I reboot my computer to let it do its thing and it goes through the repair just fine.
The problem is, when it finishes repair it reboots the system and immediately enters repair again. I've been able to get to the recovery mode where I can access command prompt to run chkdsk and again it came up clean. I ran the windows startup troubleshooter and it said it could not fix the problem and gave me a logfile with this info

Multiple times I let the computer start up and go through its own automatic repair and complete and it would just reboot. I would then manually go through command prompt and run it again just to make sure and surely enough the file was clean.
I am writing this from the same computer, but from a different operating system (Mac OS X) on a different SSD.
Any help at all would be greatly appreciated!!
