In custom loops what causes leaking after week, months, or years of use?

Improper connection between tubing and nozzles, failed O-ring somewhere.

Really the only thing that causes leaks is poor/incorrect installation and lack of common sense. Its also hard to find information/help on watercooling that will apply specifically to your situation.
