What gpu is this ?

So I came across this old gpu. I don't have the box for it nor can I put it in a pc and check the model number and so on. I would be very grateful if someone could maybe tell me the model name or the reference gpu its based on ? Here are a few pictures: http://imgur.com/a/j7hKk

They arent very high quality, but there really isnt much to this gpu. On the other side there are only 2 codes that I googled but got no results. Any help would be appreciated

I know that it is an ATI card. It is made by Inno3D. Do you have any pics of the back?

I don't know what it is. But I recommend that you install the app "google goggles" on your phone in order to find out. It's Google's visual search app. Then simply take a photo and let the app search for you. But please try to take a better quality photo; the one you uploaded is blurry.
