What increases CPU temps?

Does CPU usage, voltage, or clock speed increase heat? I was thinking voltage is what directly increases heat generation, am I wrong?

Voltage -- Watt -- Heat

Clock speed does not increase heat output, unless a voltage increase was applied in BIOS, or in software. Not enough to actually matter anyways.
