What is an original model name?

I'm trying to do my rebates but I'm not sure what the original model name is.

There's a sticker with the serial no., part no., product name, EAN, dimensions, and the weight on it. Is the original model name on this sticker or do I cut out the big name out of the box (ASRock Z97 Extreme6)?

I can't answer your particular situation, but whenever I have asked for a rebate from ASRock, I have needed to send in the original UPC (Universal Product Code) barcode from the box, which I cut out with a sharp knife. That's the only part of the box I have had to send in, and I have always gotten my rebate (so far).
