What to upgrade first?

Games like GTA V are extremely CPU limited, so your aging dual core is going to be a problem with those kinds of games. Having said that...
It looks like you must have a HD-5450 or 5470 gfx card. That is your main weak link. Even the lowly C2D E6750 CPU can keep up with more card than that. I don't know your budget, but if you have a 300W PSU, the GTX 1050 Ti would be ideal.

I would say firstly go with used core 2 quad CPU, something like Q6600 or Q8200 prices are (15-30$), GT1030 2GB GDDR5 Graphics Card(70$) will do the job and get 2 sticks of 4GB ram (40$). Good Luck fellow Gamer :)

So just this build can run GTA V on good settings? Or at least upgrade the CPU first, right?

Good advice above.

Look at YOUTUBE videos on replacing a CPU. It is not impossible, but it is not simple like installing a video card. Make sure you are willing to do the job before you buy and get the other parts you need like thermal compound and a heatsink that will work with a hotter CPU.
