You can use any rj45 keystone you want to. The key is to make sure you have the wires similar to they way they did it.

This is a pretty standard phone install with pair 1 on pins 4/5 and pair 2 on pins 3/6. I would assume VDSL since you only need a single pair for adsl.

If you are using rj11 cable to your router you can use a rj11 jack just make sure the pairs are the same...the pin numbers are different but they are still the inner 4 pins being used.

Who knows what it's wired for but jack itself does have 8 holes, and 568A/B label.

Sorry guys i should have mentioned it was for internet. The internet company came out and this is how they wired it, they installed the new rj45 jack, well i thought that's what it was. I don't have phone it's just internet. After looking at it again this is a phone jack. The back of my modem is connected through "DSL" with the green phone cable.

I was trying to extend this cable on the other side of the wall. This jack is on my kitchen wall. I would like to have this jack on the other side of my kitchen wall but about 3 feet lower. which i why i am trying to find out what type of jack this was and where to get some. What would be the best way to extend this line about 3 feet?

Oh this is easy then.

As you said, this is a plain phone jack, but disguised as a network jack with its fancy cable and big jack. U can simply cut and splice, only the 2 wires going into pins 4&5 are being used. U can find a similar jack at your local home depot, again you want to wire to the middle 2 pins, 4&5.
